Records for neurosurgeon Amos Dare, M.D. show that the claimant has been under his care since June 2007. On October 1, 2007 she underwent L4, L5, S1 laminectomy and L4-S1 posterolateral fusion (Exhibit 14F). She was then referred to physical therapy. Progress notes show that she continued to complain of pain (Exhibit 22F). On February 6, 2008 she presented to St. Mary's Hospital with complaints of back pain and tightness and was given Toradol and muscle relaxants (Exhibit 21F).

Consultative examiner StevenL. Kanner, D.O. stated on February 11, 2008 that the claimant was suffering from a chronic pain syndrome and could sit and stand for short periods of time, along with walking, lifting and carrying for short periods of time. Her grip strength was 4/5 with fine and gross dexterity maintained. She weighed 213 pounds and was 5'8" tall (Exhibit 15F). On March 3, 2008 Dr. Barron stated that Lyrica seemed to have caused weight gain and she was started on Neurontin (Exhibit 26F, page 16).

When Dr. Dare saw the claimant on August 14, 2008 she complained of left gluteal pain and occasional back muscle tightness. She was scheduled to undergo lumbosacral trigger point injection with her rheumatologist Dr. Barron. She was to continue use of her lumbosacral bone stimulator to promote optimal bone growth (Exhibit 22F, pages 1-2). Dr. Barron administered trigger point injections to the left gluteal area on August 15,2008 (Exhibit 26F, page 7). On October 2, 2008 Dr. Barron stated that the claimant was having problems functioning. She had numbness in her hands and calves and had arm and leg weakness, as well as lower back pain. Ambienand Mirapexwere started (Exhibit 26F, page 2).

In addition to her physical problems, the claimant had emotional problems and was treated for major depression and prescribed medication by psychiatrist Sneh Kapila, M.D. from July 12, 2007 through March 17, 2008 (Exhibit 18F). An assessment by Maggie C. Dante, LMHC in October 2008 indicated that the claimant was constantly in pain, which exacerbated her mental condition. She suffered from depression and mood swingsand was unable to effectively manage her stress. This also put a strain on her relationships (Exhibit27F).

On October 2, 2008 Dr. Barron reported that the claimant had back pain, hand numbness, heaviness of legs and arms and leg pain. She could sit for two to three hours in an 8-hour day and stand/walk. for less than two hours in an 8-hour day. He indicatedthat she sometimes used a cane or walker for assistance. She could occasionally lift and carry up 10 pounds (Exhibit28F).
