At the request ofthe Social Security Administration, a psychological consultative evaluation was performed by Edwina Hamilton Bell Psy.D on March 24, 2008. The claimant reported that her mother was in jail during most of her childhood and she was raised by her maternal grandfather who sexually abused her until age 14. She also reported a history of recurrent auditory hallucinations, depression, psychiatric hospitalizations, drug abuse, prostitution as well as numerous arrests. Symptoms included daily mood swings, hallucinations, sadness, anger, bulimia 3-4 x per week and difficulty sleeping. Upon mental status examination, the claimant presented disheveled and overdressed for the warm weather, had poor dental hygiene and bad body odor. She was oriented to person, time and situation but not place. She had an extremely intense affect and thought processes were irrelevant and circumstantial with loose associations. The claimant reported auditory hallucinations of her grandfather threatening to kill her if she told about the sexual abuse and responding back to him. She noted that even with medication she continued to hear voices. The claimant's intellectual functioning appeared to be below average, social functioning appeared poor and she evidenced poor insight into her current condition. Diagnosis was schizophrenia, disorganized type.

At the hearing, the claimant testified that she is unable to work because she is unable to handle the stress of a workload. She testified that she suffers from constant auditory hallucinations, telling her to kill herself or dress like a prostitute, and has been in prison. She reported that she has quit drugs but did have one relapse in 2008.

After considering the evidence of record, the undersigned finds that the claimant's medically determinable impairments could reasonably be expected to produce the alleged symptoms, and that the claimant's statements concerning the intensity, persistence and limiting effects of these symptoms are generally credible.

The undersigned notes that he has considered and given considerable weight to the opinions of the treating/examining physicians as they are substantiated by the objective medical evidence of record.

The State agency medical consultants' physical assessments and psychological consultants' mental assessments are given little weight because other medical opinions are more consistent with the record as a whole and evidence received at the hearinglevel shows that the claimant is more limited than determined by the State agency consultants.

4. The claimant has been under a disability as defined in the Social Security Act since July 20, 2006, the date the prior application was filed (20 CFR 416.920(d)).

5. The claimant's substance use disorder(s) is not a contributing factor material to the detennination of disability (20 CFR 416.935).

DECISION

The claimant has been disabled under section 1614(a)(3)(A) of the Social SecurityAct since July 20, 2006, the date the prior application for supplemental securityincomewas filed.

The component of the Social Security Administration responsible for authorizing supplemental security income willadvise the claimantregarding the nondisability requirements for these payments,and if eligible, the amount andthe months for which payment will be made.

A determination to appoint a representative payee to manage payments in the claimant's interest is recommended.
