The State Agency review physician who completed the Physical Functional Capacity form on June 9, 2006 determined that the claimant was capable of performing light work with only occasional postural limitations (Exhibit 6F, pgs. 1-8). The State Agency review physician who completed the Physical Functional Capacity form on August 3, 2006, increased the claimant's exertional capacity to medium and indicated that she could perform postural activities frequently (Exhibit 8F, pgs. 1-8). Another Physical Functional Capacity form that was completed on December 19, 2007, indicates light work activity with frequent and occasional postural limitations (Exhibit 13F).

The medical evidence shows Fernando A. Bolufer, completed and submitted a medical source statement (physical) dated February 4, 2008 in which he essentially found the claimant unable to perform the minimal requirements of less than a full range of sedentary work activity (Exhibit 21F, pg. 28).

Additional progress notes dated July 25, 2008 through January 12, 2009 from Dr. Ballesteros document continued treatment for chronic orthopedic pain (Exhibit 25F).

Jorge M. Cabrera, M.D., P.A., a board certified orthopedic surgeon, saw the claimant in consultation on January 21, 2009 for ongoing painful orthopedic symptoms (Exhibit 26F).

Turning to the claimant's mental impairment, the medical evidence shows Manuel E. Alvarez, Ph.D., P.A., a clinical psychologist, examined the claimant and completed a consultative evaluation at the request of the Social Security Administration on August 22, 2006. He reported that the claimant complained of depression and anxiety and was prescribed Lexapro and Klonopin. She told him that her symptoms began when her work status changed. Dr. Alvarez reported regarding her mood, that her current range of affect was slightly constricted. The expression of affect was appropriate to thought content. He described her mood as sad. The claimant explained to the doctor that she had some suicidal ideas approximately one month ago when she attempted an overdose. She denied any current suicidal ideation or plans. The claimant told Dr. Alvarez that her sleep was interfered with pain and anxiety and that her appetite fluctuated as did her weight. Her energy level was described as none.

Dr. Alvarez reported that the claimant believed she hears "my consciousness or GOD" saying "You may as well end it all." In general, the messages are one of hopelessness. He described the claimant as being fully oriented to person, place and time. Frank delusional material was not referenced. The claimant told the doctor that she felt watched in the past. She complained of problems with her memory and could not recall what she had for breakfast or lunch the day before, but did remember dinner. Dr. Alvarez reported that her attention and concentration seemed partial and that her insight and judgment are retained. He estimated her intellectual functioning to be within the Average range. His overall diagnostic impressions were: Pain Disorder - Chronic, Major Depressive Episode (Rule Out Psychotic Features) (Exhibit 9F, pgs, 2 & 3).
