On January 27, 2009, Sarah D. Smith, Psy.D., conducted a clinical evaluation with mental status and memory test as requested by the undersigned (Exhibit 20F). The claimant's general memory index and her ability to acquire auditory information over multiple trials are on the extremely low range (Exhibit 20F). Her low performance on general memory measures negatively affects her functioning in daily living and other cognitive domains (Exhibit 20F). She has a very significant difficulty retrieving recently learned information (Exhibit 20F). Her delayed memory performance relative to her immediate memory capabilities indicates retention difficulties in both auditory and visual information (Exhibit 20F). The claimant's ability to understand, remember and carry out simple instructions is moderately affected. However, her ability to understand, remember and carry out complex instructions is extremely affected. Likewise, her ability to make judgments on complex work-related decisions is extremely affected (Exhibit 20F). Dr. Smith does not believe the claimant is capable of managing her funds at this time. Finally, Dr. Smith opines the claimant's impairment is unlikely to improve and her prognosis is poor. As this opinion is supported by the medical evidence and is consistent with the record as a whole, it is entitled and accorded great weight (Social Security Ruling 96-6p).

The State agency medical consultants' physical assessments and psychological consultants' mental assessments are given little weight because other medical opinions are more consistent with the record as a whole (Exhibit 11F). Furthermore, the State agency consultants did not adequately consider the claimant's subjective complaints. In sum, as aforementioned, Dr. Dadi has had an opportunity to personally evaluate and examine the claimant on several occasions and is therefore better suited to provide an accurate depiction of the claimant's impairments.

At the hearing, the claimant testified she has suffered three heart attacks. In 2005, she suffered back to back heart attacks and pulmonary edema. She recounts she started experiencing memory loss following this last incident. Specifically, she forgets words, looses track of conversations and forgets what she is doing. There are times she drives to nearby places and sometimes forgets how she got there. In 2005, she was attending school to become a medical assistant; but, after her attack she was asked to leave because she was unable to keep up with the rigors of the curriculum. The claimant used to be a veterinary assistant but she is no longer able to perform that job. She explained that to be a veterinary assistant you have to be able to lift and hold the animals and run tests; however, she has trouble lifting more than ten pounds. In fact, she explained that she is even unable to perform every day activities. She related how she experiences such fatigue while walking, that even going to the mail box or taking the garbage out leaves her breathless.
